ALLEGED SEXUALLY ABUSED (A.S.) CASES MANAGEMENT PROTOCOL
1. All children with vaginal discharges are potential victims of A.S.A.
Children with proven gonorrhea on vaginal swabs are A.S.A. victims until proven otherwise and must be investigated.
2. Suspect A.S.A if there is vaginal bleeding or trauma, but remember straddle injury or prolapsed urethra.
3. When child abuse is suspected, the patient should be interviewed and examined by a fully registered doctor. (Not an Intern.)
4. request the presence of a Police Officer, which is essential if forensic specimens are to be collected. In Kingston between 7:00 a.m. and 10:00 p.m. Contact center for Investigation of Sexual Offence and Child Abuse (CISO & CA) 926-6538 or 926-0479. Otherwise contact the police in the area in which the child lives. If unsuccessful, call 119.
FORENSIC EXAM
Specimens are only useful if done within 48 hours of sexual contact.
These must be taken in the presence of the police.
All specimens must bear the name, date, time contents, doctors’ signature.
Specimens must be placed in sealed envelopes and handed directly to the police.
A detailed receipt is to be received from the police with the officers’ name, badge number and division.
The necessary swabs and glass slide should be provided by the police.
Take four vaginal swabs: two for sperm identification and one to make a smear on the glass for staining and one for culture (For G.C.)
Swab perianal/vulva area for both male and female and place in Stuarts media to detect G.C.
Flush vagina with 10mls of distilled water through a Foley catheter; give washings to the police.
Take swabs in all cases, both and female and place in Stuart’s media to detect G.C
Take blood for VDRL, HIV, Hep. B for baseline values repeat in 3/52 for VDRL status and 3/12 for step B HIV
