DIABETES PREVENTION: AN EVIDENCE-BASED REVIEW

Type 1 Diabetes
Family history of 1 diabetes and autoantibodies to the islet antigens, insulin (IAA), glutamate decarboxylase (GADA) and the protein tyrosine phosphatase-like protein IA-2 (IA-2A) are many predictors of type 1 diabetes.

Studies to prevent immune-mediated destruction of the pancreatic islet beta cell have included parenteral insulin therapy, which is postulated to act metabolically or immunologically.

The results of the Diabetes prevention Trial – Type 1 (DPT-1) and the European Nicotianarnide Diabetes Intervention Trial (ENDIT) are presented.

Immune intervention with cyclosporine, for instance have proven that this possible but the unwanted effects are many. Interventions earlier in the immune pathway such as anti CD3 antibodies have been encouraging.

Type 2 Diabetes
Studies on the prevention of Type 2 Diabetes in subjects with impaired glucose tolerance have been encouraging. Results of studies on lifestyle intervention and the pharmacological agents such as Melformin (Diabetes Prevention Program Research Group), Acarbose (The STOP IDDM Trial) and Trohtaz and TRIPOD study) are presented.
